Use Preview’s thumbnail sidebar to put selected supporting pages after an application.
Assemble a working copy
Open the destination PDF and choose File > Duplicate. In both PDFs, choose View > Thumbnails. Drag needed thumbnails from the source to the insertion point in the duplicate. Command-click nonadjacent pages, then drag thumbnails up or down to set the order. For example, put the two-page application first, one proof-of-address page next, and the instruction page last: four pages total.
Verify the result
Reopen the finished PDF and check the page count, first and last pages, and the inserted-page order. Confirm that no extra attachment slipped in. Keep the originals until the reviewed copy is ready; automatic saving makes an untouched source useful.
When Preview refuses an edit
If a PDF requests a password or disallows changes, ask its owner for an editable copy or permission. Do not bypass the restriction. For an extract that should stay separate, see how to save selected pages. Apple recommends duplicating first in its combine guide; Macworld supports the insertion and reorder steps.
